How can I examine the record of all badges issued to me?

You can review your entire badge history (since the current badging system went into production on September 20, 1999) through the Laboratory's Data Warehouse.

Each person with an active EIS record will be able to access his or her own badge history - this includes not only Laboratory workers but visitors and uncleared foreign nationals. Group managers have the authority to access the badge history of everyone in their Group, and are permitted to assign that authority to non-managers.

To access your badge history, you will need:

  • A Laboratory workstation, or ability to dial-in to your Lab account from a non-Lab computer;
  • An active EIS record;
  • A administrative-level Token Card that authorizes you to access your data;
  • DWALL authority.

To run the Badge History Data Warehouse report:

  1. Go to the Laboratory (internal view) home page, and pulldown the Select a Shortcut window
  2. Select Data Warehouse
  3. At the Data Warehouse home page, click on Select a Report
  4. If you are not currently logged into the Data Warehouse, you will be prompted to use your Token Card and authenticate yourself by entering your Z number and then your password
  5. At the next page select (click on the tab) or make sure you are in the folder labeled Personnel
  6. Go to the category labeled Badge Office Information
  7. Select Badge History Report
  8. Fill in either your name or Z number (preferably the latter, but do not fill in both - make sure other fields such as Division Code and Organization Code are blank
    • Whenever you enter letters, they must be UPPERCASE
    • If you do not know how your name, in full, appears, and you wish to search on your name, enter part of your name followed by an asterisk or you will not retrieve your name
    • If you search by badge number (the number in the upper right corner of your badge) you must:
      • Include both the letter and numerical portion
      • Convert the two-letter prefix to a numeric equivalent - the conversion is based on the position of the letter in the alphabet, e.g., badge prefix CR = 0318 so that CR12345 should be entered as 031812345;
      • Note the following exceptions - enter:
        • EX as 0000
        • FN as 0600
        • FT as 0600
        • OV as 1500
        • TM as 2000
        • UV as 1500
  9. Click on the Create Report button
  10. Some of the important information you will see is:
    • Badge serial number
    • Badge issue date
    • Badge expiration date (if applicable)
    • Badge status (ACTIVE, DESTROYED, LOST/STOLEN, STORED, etc.)
  11. A badge listed with a badge status of ACTIVE is a current, valid badge in good order and should work; a badge with status NOT ACCOUNTED FOR must be returned before the badgeholder may obtain any further badging services. PLEASE NOTE:
    • Data Warehouse reports are current only through the previous day - if there have been any changes in your badge history that occurred on the day you run the report, they will not be reflected;
    • If you want to know your badge and access status in "real time," run the Badge and Access Status Report, not the Badge History Report. The Badge and Access Status Report is available from the Badge Office's Web site.

A Lab manager can access the badge history of everyone assigned to his or her organization. If you do not have your own Token Card, you can ask your Group Office to obtain your badge history for you.
