There is no ñone size fits allî answer to how to get an EIS record corrected.æ The answer depends primarily on two factors:
If your ñEmployee Typeî field in EIS is LAB, that is, you are paid directly by the Laboratory (e.g., UC employees, those in a student program, associates) a change request must be submitted by your Laboratory organization, using the Personnel Action Form (Lab Form 1071), to the BUS-1 Payroll Office (667-4594). Following this procedure, you can have your Social Security Number (SSN), date of birth (DOB), citizenship, etc., corrected. If your ñEmployee Typeî field is any other category than LAB, whom you contact to report an error in a critical field (SSN, DOB, citizenship, employer name and termination date) and request a change becomes a bit more complex. Follow these guidelines:
