A temporary badge can be issued to you if you have a LANL-issued DOE Standard badge (your "regular" badge) but forgot to bring it to work or cannot locate it. The temporary badge is color-coded at your clearance level, just like your regular badge. (A red temporary badge is issued if you are an uncleared foreign national assignee.) A temporary badge has your name and Z number on it, but no picture. If you present the badge to a guard, you will be asked for valid photo identification. Your temporary badge will work just like your regular badge for going into a security area through an automated access control station. (For uncleared foreign national assignees, the red temporary badge must be reported to CCN-4 before it will work at those reader locations the badgeholder is approved for. Contact CCN-4 by telephone (667-7840), Fax (665-1634) or e-mail (badgereaders@lanl.gov) Depending on the circumstances, a temporary badge can be issued for one or more days, but never for more than 7 calendar days (generally equivalent to 5 workdays) from the date of issuance. You cannot be issued temporary badges consecutively - the situation for which you were issued the temporary badge must be resolved within the period for which the temporary badge is valid. During the period your temporary badge is active, your regular badge will be inactive. To re-activate your regular badge, you must return the temporary badge to the Badge Office, in person, along with your regular badge, by the expiration date. Do not leave your temporary badge with a guard, at a guard post, return it through the mail, or put it in the Badge Office Drop Box. If by the time your temporary badge expires you cannot locate your regular badge, you must report it lost, by filling out and signing an affidavit, Lab Form 1672 (Notification of Permanent Inactivation of Badge). Once you have completed, signed, and submitted this form to the Badge Office, a new permanent badge can be issued to you. Every badgeholder at the Laboratory who qualifies for a temporary badge can be issued 2 in a 12-month period without consequences or higher level approval. If there is a 3rd request within 12 months, the individual's immediate manager (usually a Group Leader) needs to approve issuance of a temporary badge in writing. A 4th occurrence requires Division Director approval plus a corrective action plan for reducing the number of times a temporary badge has been requested. A 5th occurrence within the same 12-month period will result in the initiation of a security investigation, possibly leading to an infraction, and no badge will be issued while the investigation is being undertaken. Each replacement of a badge reported lost is counted as a temporary badge. Thus, after one reported lost badge and one temporary badge, the next temporary or lost badge within a 12-month period requires a written request from the Group Leader. A temporary badge issued while determining if a badge is lost is not counted against you. PSAP Participants: If you are a participant in the Personnel Security Assurance Program (PSAP) and your regular badge has a yellow numbered PSAP strap, you must go to the PSAP Office after obtaining a temporary badge at the Badge Office. The PSAP Office will issue you a yellow PSAP temporary badge valid for one-day only. If the temporary badge issued by the Badge Office is valid for more than one day, you will need to go to the PSAP Office every day to re-validate your use of the PSAP temporary badge.
