Procedure: If Your Badge is Lost or Stolen

BACKGROUND

  • If you discover that your badge is lost or stolen, you are required to report it as such immediately.
  • If you have misplaced your badge and believe it will turn up, you should not report it as lost. Instead you can be issued a Temporary badge (for up to one week or five working days). If at the end of that time you still cannot find your badge, you must report it as lost.

HOW TO REPORT A BADGE AS LOST OR STOLEN

  • Submit a Lab Form 1672 (Notification of Permanent Inactivation of Badge) to the Badge Office in person – if you do not fill this form out before bringing it in, the Badge Office will supply you with a blank copy to fill out and sign
  • There are two important questions you must answer:
    • Was your badge stolen? If the answer is YES, you must report this immediately to the S Division Office of Security Inquiries, 665-3505.
    • Did your badge include a non-removable yellow strap indicating that you are enrolled in PSAP? If the answer is YES, you must proceed to the PSAP Office (Otowi Building - 2nd floor) once your replacement badge is issued (see below) to report your accountable yellow strap as lost and obtain a new strap

REPLACING YOUR MISSING BADGE

  • The status of the missing badge will be changed immediately by the Badge Office to LOST/STOLEN and will immediately cease working in any Laboratory badge reader, even if found – this is a permanent status change, so that the badge reported lost or stolen can never be re-activated.
  • You will be issued a replacement badge, with a new picture and a new badge number – if your hand geometry template was already enrolled for security area access control readers, you will not have to be re-enrolled.
  • If you have a combination of more than two Temporary badge issuances and/or Lost Badge replacements in less than 12 months, you will need to get management authorization before a badge will be issued. A Laboratory memorandum describes the specifics.

IMPORTANT: If you locate a badge you have already officially reported lost (following the procedures above) you must still bring it in to the Badge Office for physical destruction.
