What to do When Your Name Changes

Applicability

Laboratory workers holding a DOE Standard Badge. The procedure differs depending on whether a clearance is involved.

Definition of a Change of Name

A change of name includes any action taken that legally changes your name, such as marriage, divorce, court-granted name changes.

What to Do

If you hold a clearance:

  • Request your Group Office (or the organization responsible for updating your Employee Information System record) to process a name change through HR Division.
  • Complete Lab Form 1705 ((Name Change Report) and fax it to the S-6 Clearance Processing Team (Fax: 665-2919)
  • Reporting requirements to the S-6 Clearance Processing Team:
    • If you have legally changed your name through a court action, submit Lab Form 1705 within 5 working days
    • If you have changed your name through marriage or divorce, submit Lab Form 1705 within 45 days of the date your name changed
  • Once notified by the Clearance Processing Team that your name change has been processed, come to the Badge Office to turn in your old badge and receive a new badge. Your old badge will stop working in the Laboratory's security access control system as soon as the name change has been processed.
  • Do NOT come in for a new badge until notified by the Clearance Processing Team that your name change has been processed.

If you do not hold a clearance:

Report the name change to your Group Office or whatever organization is responsible for updating your personnel record in the Employee Information System (EIS). You should come in to the Badge Office to turn in your old badge and receive a new badge shortly after your name change has been processed in EIS.
