Outgoing Visits to the AWE (United Kingdom)
|
For Laboratory travelers going on classified visits to the United Kingdom's
Atomic Weapons Establishment (AWE) the following must be successfully
completed, over and above those services provided by the Badge Office:
- The traveler and the AWE host agree upon visit, dates, and under which
JOWOG's auspices the visit will be conducted.
- The UK host notifies the US/UK Liaison Office at AWE.
- The traveler notifies LANL's US/UK
Administrative Coordinator of pending travel and dates.
- The UK Embassy in Washington, DC, assigns a VISAM number, which the
LANL US/UK Administrative Coordinator forwards to the traveler.
- The traveler completes and submits to the LANL Travel Office Laboratory
Form 1512 (Request
for Approval of Foreign Travel) - it
is important to include the VISAM number
if it has been assigned (if a VISAM has not been assigned at the time
the form is filed, indicate "under auspices of JOWOG number __" .
- The LANL US/UK
Administrative Coordinator sends the traveler instructions on the
additional requirements for filling out DOE Form
F 5631.20 (Request for Visit or Access Approval) for an AWE
visit
- The traveler completes this form in accordance with the Guidelines,
plus any special instructions from the US/UK Administrative Coordinator,
and then submits the form to the Badge Office. The form must
include the following items (which are not specifically requested but
must appear):
- The VISAM number;
- Place of birth (city, county, state or country);
- Home address;
- Job title;
- Passport number and expiration date.
- Along with providing the special instructions for filling out DOE
Form 5631.20 in Step 6 above, the US/UK Administrative Coordinator will
also include instructions for filing the UK trip report.
- Within 30 days of completing the trip, the traveler must file a trip
report in accordance with the instructions.
